THE FREUDIAN SCHISM SIR,—Why should there be any controversy about

the medical qualifications of the psychoanalysts Melanie Klein and Dr. Segal? If one is to treat prob- lems of psychological origin, a knowledge of basic physiology is useful for the understanding of psychosomatic disorders. A knowledge of organic psychoses and of endocrine and nutritional dis- orders is necessary if symptoms are to be correctly diagnosed. One does not have to qualify as a physician to gain this knowledge.

The essential qualification is an understanding of the dynamics of human behaviour, especially un- conscious phenomena, and of modern psychotherapy. Since this is not part of the medical curriculum, there is little point in a psychotherapist having a medical degree. On the other hand, most physicians could usefully learn something of psychodynamics and psychotherapy. This would reduce the common problem of patients with psychogenic disorders being given inappropriate treatment by physicians.
